Directions
We are located at the intersection of Sunrise Highway and Franklin Avenue, adjacent to the Valley Stream Train Station. A metered parking lot is available for patient use directly in front of the office.
By Bus:
N1: (LIRR Stop) is the closest stop to the office and is located underneath the train trestle.
N4: (Franklin Ave. Stop) Head south on Franklin Avenue towards Sunrise Highway, make a left at Sunrise Plaza.
